Using Telephonic Case Management to Treat Homebound Adults with Physical & Mental Disorders – Montefiore Medical Center, Bronx, NY

Studies show that over 2 million elderly Americans are homebound and that number is growing.  Many of them have not only physical disorders but psychiatric or addiction disorders.  Cognitive disorders, depression and anxiety are twice as prevalent in homebound older adults as non-homebound.

This presentation & discussion will illustrate how telephonic case management can successfully screen for mental disorders like depression and trigger in home assessments of home bound older adults by partnering with a certified home health agency and a colocated geriatric psychiatry program. During this session, Mirnova Ceide, MD, assistant professor of geriatric psychiatry and medicine at Montefiore Medical Center, Bronx, NY, and Janice Korenblatt, LCSW Director of Social Work at Montefiore Home care will describe comprehensive and practical plans to provide mental health care to home bound patients and minimize unnecessary health care utilization.

Dr. Ceide and Ms. Korenblatt will use case examples from the Montefiore Home Care Geriatric Psychiatry Program to illustrate how case management, home care and psychiatry were able to collaborate across the health network to maintain homebound older adults safely in the community. Webinar participants will learn the cost and replicability of this work, the next phase of the initiative, and most importantly how it’s improved outcomes and reduced the overall cost of care.
