A Missing Link

  • Medical model prevails - limited (or no) mention of function, functional gains, or how medical conditions are impacting functional performance
  • May not be included in Patient Assessment Instrument (PAI)  scoring or has limited input–impact both payment and justification of rehab necessity for admission (72 point PAI scoring guideline)
  • Often not able to document functional component because forms do not allow

Example

“Pt. cont. to attempt to use amputated LE secondary to phantom sensations, safety reminders provided. She has requested material to “stuff” leg of pants to replace LE during dressing task. Alternatives provided. Note to OT for update & additional training. Education provided regarding phantom sensation, referral toamputee support group discussed, Pt. would like services. Social Worker notified, referral made.”
