These percentages are related to a profit/stock relationship rather than a governance relationship.
For column (e), only physicians employed by or physicians who had staff privileges with one or more of the organization's hospitals should have their percentages
included. If a physician is also an officer, director, trustee, or key employee, their percentage should be included in column (d) rather than column (e). Do not
include their percentage in both columns.
