Please be aware that these criteria reflect a vision for health care that is well described in the Institute of Medicine report (IOM), Crossing the Quality Chasm. 

This report describes six aims (safety, patient-centeredness, effectiveness, efficiency, timeliness, and equity) as the basis of a comprehensive quality-oriented health care system. 

This award will honor hospitals that are committed to and have made demonstrable progress toward making this vision a reality.
