Leadership Award Background

The Rural Hospital Leadership Team Award from the AHA Rural Health Services honors the leadership team who have guided their hospital and community through transformational change on the road to health care reform. The team will have displayed outstanding leadership and responsiveness to the community’s health needs and demonstrated a collaborative community process that has led to measurable outcomes.

The team will receive a stipend for 2021 of $1,500 to defray the team representative’s expenses to attend the 2022 AHA Rural Leadership Conference in Phoenix or the 2022 AHA Annual Meeting in Washington, DC. The AHA will work with the honoree and finalists to provide national recognition of their achievements through media releases and announcements and feature stories in our national media channels.

Applicants must be the chief executive officer, president or administrator of an AHA member hospital located in a rural area. We encourage applications from hospitals that demonstrate they are on the road to health care reform. The move from a fee-for-service to a value-based environment is prompting hospitals to focus more intently on quality and embrace population health management, defined as both “attributed” and geographic populations. Hospitals are becoming more clinically integrated and are making concerted efforts at better coordinated care. They also are dedicated to goals for improving the health of the community through increased access to primary care, appropriate admissions and reduced readmissions, along with making measurable gains in improving outcomes of care and reducing harm.

We request a description of your efforts on one or more of the following initiatives:

  • Coordinated care
  • Improved population health through disease specific or public health interventions
  • Clinical integration
  • Accountable care, bundled payments and managed risk
  • Advances in access to care
  • Increased capacity of clinical workforce
  • Extended or expanded access to coverage and
  • Improved outcomes

Applications must be submitted by August 30, 2021.
Award recipient and finalists are notified in November.
