We are also aggressively pursuing a public policy environment that will protect access to care, advance innovation, and invest new resources in rural communities.

AHA Statement to House Ways & Means Committee on Access to Health Care in America: Ensuring Resilient Emergency Medical Care
Many hospitals, including those in rural and underserved areas, continue to experience unprecedented challenges that jeopardize access and services. These include workforce shortages, high costs of prescription drugs, and continued severe underpayment by Medicare and Medicaid. Given that rural hospitals tend to be much smaller, patients with higher acuity often travel or are referred to larger hospitals nearby. As a result, in rural hospitals, the acute care occupancy rate (37%) is less than two thirds of their urban counterparts (62%). On March 18, in a letter AHA offers a series of proposals and suggestions for the Ways and Means Committee to consider as it seeks to strengthen emergency medical care for patients across the country.

The House of Representatives March 6, passed extensions of key rural hospital provisions as part of a legislative package with six appropriations bills, including an extension of the Medicare-dependent Hospital (MDH) and enhanced Low-volume Adjustment (LVA) programs.

However, these hospitals continue to experience ongoing challenges that jeopardize their ability to provide local access to care and essential services. These include continued workforce shortages, emerging challenges posed by commercial and Medicare Advantage plans, soaring costs of providing care, severe underpayment by Medicare and Medicaid, and an overwhelming regulatory burden.

The AHA continues to work with Congress and the Administration to enact policies to support rural hospitals. We also are working to support a public policy environment that will protect access to care, advance innovation and invest new resources in rural communities.

Hospitals and health systems play critical roles in preserving access to care for patients in rural America. They have increasingly stepped up to fill voids in access to care by reinvesting through access points like hospital outpatient departments (HOPDs). These sites of care provide essential services for rural and low-income communities. Oftentimes, hospitals have been a lifeline for struggling rural physician practices — helping to keep their doors open. HOPDs, meanwhile, have remained convenient high-quality key points of access for patients with more complex care needs
