By integrating the Age-Friendly Health Systems 4Ms Framework in its Geriatric Fracture Program, Cedars-Sinai Medical Center is providing valuable care to older adults. Additionally, the Cedars-Sinai team established telehealth visits for older adults during the COVID-19 pandemic, ensuring the 4Ms are practiced with each patient. OB Nest Redesigns Prenatal Care for Low-risk Pregnancies

Mayo Clinic developed the OB Nest program to optimize prenatal care for low-risk expectant mothers and transform care from a medicalized model to an innovative wellness model. Patients enrolled in the program report being more satisfied with the care received and less prenatal stress. Read more.

Standardization Results in Enhanced Value and Improved Outcomes

Multidisciplinary teams at the University of Tennessee Medical Center used lean methodology to create evidence-based care plans throughout any episode of care, including the ED, outpatient, inpatient, post-acute and home care. The medical center also implemented a comprehensive medication analysis for patients receiving nine or more medications. By standardizing clinical pathways and order sets, UTMC has reduced readmissions, lengths of stay and costs. Read more.

Critical access hospital engages paramedics to improve care through home visits

Through the Peace Harbor’s Mobile Integrated Healthcare program, a dedicated paramedic checks on patients at home for their safety and provides preventive care to reduce their ED visits, avoidable readmissions and ambulance trips. Home hospital program provides quality care at lower cost

Through its home hospital program, Brigham Health patients receive hospital-level care in the comfort of their own homes. Patients in the program experienced fewer clinical interventions, more physical activity and comparable patient satisfaction scores as those being cared for in the hospital. Hartford HealthCare – Connecticut
Logistics Center Coordinates Care Among Six Hospitals

Hartford HealthCare instituted a centralized logistics center to maximize patient flow among its flagship academic medical center in Hartford and its five other hospitals in Connecticut. Since its launch in October 2017, the center has, among other improvements, decreased the time it takes to get patients to the next level of care and increased the number of patients staying within the HHC system.
