AHVRP webinar documentIn this session, the speakers will discuss the concept and launch of the "Discharge with Dignity" program for unfunded/indigent discharging patients. They will share the concept and process of working with Case Management and Care Transitions team to provide DME and other necessities to patient population vulnerable to readmission. Example of community awareness projects will be highlighted, including fundraising fashion show, homeless outreach project, battery and light bulb recycle projects and acting as "Cool Zone" during warmer weather periods.

Learning Objectives:

  • Partner with Nursing Units and Case Management to "Discharge with Dignity"
  • Assist Care Transition Team with items for vulnerable patient population
  • Increase hospital positive perception in the community.
