Case Studies https://www.aha.org/ en Role of Hospitals: Cheyenne Regional Medical Center https://www.aha.org/role-hospitals-Cheyenne-regional-medical-center-research-innovation <span class="title">Role of Hospitals: Cheyenne Regional Medical Center</span> <span class="uid"><span>oronen_drupal</span></span> <span class="created">Jun 21, 2022 - 12:08 PM</span> <div class="body"><div class="container"> <div class="row"> <div class="col-md-9"> <div class="row"> <div class="col-md-7" style="margin-bottom: 10px; float: left; padding-right: 10px;"> <p><img alt="Person facetiming on their phone with a healthcare worker who is wearing a blue shirt with a face mask and gloves" data-entity-type="file" data-entity-uuid="" src="sites/default/files/2022-06/stc-cheyenne-medical-center-700x532_0.jpg" class="align-left" /></p> </div> <p>Like many hospitals across the country, Cheyenne Regional Medical Center in Laramie County, Wyoming, was seeing the effects of the nationwide opioid crisis – and had to change its approach after the COVID-19 pandemic made the situation worse.</p> <p>In the fall of 2020, CRMC’s population health division launched a three-month campaign that covered multiple topics dealing with opioid use disorder: how to get help, how to talk to providers about alternatives to opioids and the importance of stigma reduction.</p> <p>“Opioid addiction is a medical disorder, not a character flaw,” said Angela Vaughn, CRMC’s community health project manager. “As a first step, it’s important to educate and encourage people to consider alternatives to opioid use for pain management. But if someone has become addicted, they need to know where to turn for help.”</p> <p>The campaign, which was funded by a 2018 State Opioid Response Grant, also focused on educating the public about the symptoms of opioid addiction, helping someone who has overdosed and stigma reduction. Information was posted on the CRMC website, and wallet-sized cards showing how to help someone experiencing an overdose and listing available local resources.</p> <p>As the pandemic wore on, opioid and other substance abuse disorders were climbing everywhere, including Wyoming. CRMC responded by launching a class on pain management and addiction specifically targeting first responders. The “Alternative to Opioids” class teaches EMTs, emergency room staff and other first responders about non-opioid pain relief. It can take as few as five days for a person to become addicted to opioids; by teaching first responders to use alternative responses to pain, some opioid use disorders may be stopped before they begin. Their innovation work is focused on five areas:</p> <ul><li>Big data and the internet of medical things</li> <li>Community health and social determinants of health</li> <li>Genomics and precision medicine</li> <li>Next generation primary care</li> <li>Radical efficiency</li> </ul></div> <div class="row"> <div class="col-md-12"> <p>One new tool from the OSF Innovation Data Science and Advanced Informatics Lab is Community Connect, developed to identify people at risk for poorer health outcomes and help connect them to resources and ongoing monitoring. OSF Community Connect was first used by OSF community health workers who were reaching out to COVID-19 patients to assess their conditions and direct them to providers and educational resources as needed.</p> <p>With state funding aimed to improve Medicaid services, the digital platform is being expanded for use by clinicians, community-based organizations and others for treatment and preventive care. OSF OnCall Digital Health, a virtual platform that offers 24/7 access to essential health services, has adopted Community Connect for remote patient monitoring and breast cancer screening outreach — just two examples. That number will rise as the baby boomers age. The study also shows that the health of caregivers is significantly declining compared to just five years ago, which negatively impacts their ability to care for others.</p> <p>Community Memorial Health System located in Ventura, California, partnered with Channel Islands University to create a caregiver navigator program. The goal is to support and integrate family caregivers as part of the health care team. It provides resources and easy access to programs that ease the burden of caregiving and let caregivers know they are not alone. In 2020, the health system invested more than $126 million in a wide range of community health improvement activities, including increased education and access, assistance related to the COVID-19 pandemic and to regional wildfires, health-related research, training for health professionals and grants to local nonprofits in support of health initiatives. Community benefit services include immunizations for children in low-income households, health screenings transportation vouchers for appointments, housing for families in need and violence prevention programs.</p> <p>One example of Samaritan Health Services’ investment in community is its partnership with Jackson Street Youth Services. With funding from the health system, Jackson Street Youth Services offers adolescents a safe place to live and resources to work through crises. Opened June 2020, the Cletus Jeckering Family Center of Care – Ohio City Health Center offers health services to the school’s nearly 700 students, as well as their families and the entire community.</p> <p>Located on the school campus, the center offers primary care, urgent care and support services, including behavioral health, nutrition and social work. In addition, MetroHealth provides health education and wellness programming for students and families as well as professional development for school staff. With this range of services, care teams can work together and provide seamless, integrated care to the community.</p> <p>Many health care providers and staff speak English and Spanish fluently, so full-service bilingual care is available for Spanish-speaking patients and families. The health system incorporates age-friendly care into annual wellness visits for older adult patients and includes age-friendly initiatives as part of its annual strategic and financial planning.</p> <p>An age-friendly task force meets monthly to look for ways to incorporate best practices across the continuum of care. This task force includes several inpatient and outpatient departments: outpatient chronic care management team, home health, behavioral health, physical therapy, emergency services, volunteer services and radiology. The team also reached out and included community partners that focus on the care of older adults, including the Council on Aging for Hendersonville County; local, government-run health care providers; and community colleges.</p> <p>After thoughtful discussion of what an ideal health care system focused on the aging population would look like, the age-friendly task force created a vision board. In a short amount of time and with dedicated coordination, the majority of those dreams and goals became reality. As a result, the emergency department is now certified as a geriatric ED at the bronze level by the American College of Emergency Physicians — the first such designation in the region. To achieve that goal, the leadership team set out to learn what health equity looks like in the health system’s communities and measure how well the organization is providing equal access to health care for all of its patients.</p> <p>As a result of this initial work, Allina Health discovered that colorectal cancer screening rates among the health system’s Blacks and African Americans were extremely low compared to other patient populations. The problem: low-literate individuals and culturally diverse individuals with limited English proficiency faced great difficulties in understanding health information and navigating the health care system. When health care education materials and other literature are not available in a patient’s first language nor easy to understand, health care screening rates go down.</p> <p>To address this disparity, Vivian Anugwom, director of health equity, led a team that took a deeper dive into the education materials on colorectal cancer screenings that the physicians provide to patients. The team adapted these tools to be more inclusive by addressing language, literacy and cultural barriers.</p> <p>Community health workers who speak multiple languages were enlisted to follow up with patients struggling with language, literacy and cultural differences and answer questions after a provider visit. The community health workers are key to bridging the communication gap and improving access.</p> <p>Moreover, Allina Health recently released its inaugural diversity, equity and inclusion (DE&amp;I) report, which tells the story of the significant strides the health system has made in building a framework to make DE&amp;I a priority. The report details the diversity representation of its colleagues and the health system’s momentum of fostering an inclusive and equitable culture to improve the health of all people it serves. The leadership team at the hospital in Yonkers, New York, has made providing age-friendly care a priority. They know that providing care that is age friendly helps older adults enjoy a better quality of life.</p> <p>Throughout the journey to become an <a href="https://nam11.safelinks.protection.outlook.com/GetUrlReputation" target="_blank">Age-Friendly Health System</a>, the St. John’s Riverside team implemented an evidence-based approach called the 4Ms framework: what matters, medications, mentation and mobility. To effectively implement the 4Ms, an age-friendly multidisciplinary committee was formed, with representatives from nursing, medicine, pharmacy, physical therapy and IT</p> <p>Here’s how St. John’s Riverside Hospital has adopted and integrated the 4Ms.</p> <p>What matters: The hospital’s social work and case management teams conduct daily rounds with every older adult patient to discuss goals of care, special needs and discharge planning. Reponses are documented in the electronic medical record and recorded on a white board located at each patient’s bedside.</p> <p>Medications: All physicians and nurse practitioners speak to patients and their families and develop a plan of care which they document as part of the EMR. Care teams perform medication reconciliation on admissions, and comments about medications to avoid in patients age 65 and older are flagged.</p> <p>Mentation: Since 2019, nurses have performed mental status assessment and neurology checks when ordered on admission of a patient. Tools for delirium screening also are used.</p> <p>Mobility: St. John’s Riverside has an established physical therapy program, and the nursing staff uses a bedside mobility assessment tool when patients are admitted. The hospital has now increased focus on physical therapy consults for all patients age 65 and older when appropriate, which includes writing physical therapy goals on white boards.</p> <p>After implementing age-friendly care strategies, the hospital’s IT department developed a centralized dashboard for every patient age 65 and older; it’s called the Age-Friendly Clinical Panel. It’s an unacceptable reality because these deaths can be prevented with proper health care before, during and after pregnancy.</p> <p>That’s the message coming from the Hackensack Meridian Health’s Maternal Health Task Force, a blue-ribbon panel of internal and external experts dedicated to improving maternal health outcomes. The task force’s goal is to make New Jersey the safest state in the nation to have a baby. It’s a critical mission as data shows New Jersey is one of the most dangerous states for women to give birth – with the fifth highest maternal death rate in the country. In a <a href="https://njbmagazine.com/njb-news-now/hackensack-meridian-health-to-form-maternal-health-task-force/" target="_blank">press statement</a> about the task force, Robert C. Garrett, CEO of Hackensack Meridian Health, said, “Our mothers deserve better. As New Jersey’s largest health network, Hackensack Meridian Health is dedicated to changing that outcome in New Jersey.”</p> <p>The task force builds upon HMH’s work to improve maternal health and raise awareness in the communities it serves. Focus is on education, collaboration and communications. The health system’s maternal fetal medicine physicians specialize in high-risk conditions in pregnant women and their babies.
