Creating Age-Friendly Health Systems

The John A. Hartford Foundation and the Institute for Healthcare Improvement, in partnership with the AHA and the Catholic Health Association of the United States, developed the 4Ms Framework for Age-Friendly Care, an evidence-based model to support the needs of the aging community in a clinical setting. The 4Ms are: What Matters, Medications, Mentation, and Mobility. This webinar outlines the importance and impact of the 4M Framework, in addition to highlighting bright spots in the field integrating Age-Friendly care into their hospitals and health systems. Christiana Care Health System shared their story of becoming an Age-Friendly health system, and why this movement was necessary for their patient population.
