Evaluating Population Health Programs: Fundamentals and Key Components

At the end of 2018, the AHA, along with technical assistance partner RAND, launched the Hospital Community Cooperative (HCC), a program designed to bring hospitals and community based organizations together to drive powerful changes and improve health outcomes through strategic partnerships. One of RAND’s roles in the HCC program was to help support teams in designing and conducting their own process and evaluation outcomes for their respective projects. This webinar discusses RAND’s insights on how to get started on evaluating place-based projects like those implemented by HCC team members.
Attendees Will Learn:
- Why evaluation is important to consider at every phase of a project (especially in planning).
- Be able to describe the essential components of an evaluation including a Logic Model, evaluation questions, and measures.
- Understand differences between process and outcomes evaluations.
- Know how to troubleshoot barriers to data collection and analysis.
