The Institute for Safe Medication Practices is alerting hospitals, ambulatory surgical centers and anesthesia professionals about the potential for dangerous mix-ups due to look-alike packaging for the drugs Bloxiverz (neostigmine) and Vazculep (phenylephrine) made by Eclat Pharmaceuticals. In the past three months, ISMP’s National Medication Errors Reporting Program has received eight practitioner reports expressing concern about the packaging. To prevent mix-ups, ISMP recommends keeping supplies of the drugs widely separated, alerting staff to the potential risk of confusion, and verifying product selection, among other actions.
