Hospitals and health systems, health care practices, community-based organizations and others can apply through Nov. 3 to participate in a revised Track 1 funding opportunity for the Accountable Health Communities Model, the Centers for Medicare & Medicaid Services announced today. The five-year model will provide funding to implement promising approaches to raise beneficiary awareness of available community services, help those at high-risk access services or optimize community capacity to address health-related social needs. Track 1 will focus on increasing beneficiary awareness of available community services through screening, information and referral. For details, see the funding opportunity announcement. CMS will host a Sept. 14 webinar on the revised track and model application process. To register for the webinar, at 2 p.m. ET, click here.
