A new AHA report warns that access to care is increasingly at risk in rural communities as workforce shortages, financial pressures and policy changes threaten hospitals’ ability to maintain essential services.

The report finds that more than 46 million Americans rely on rural hospitals that are confronting longstanding challenges, including low patient volumes, geographic isolation and inadequate reimbursement, while also facing significant Medicaid reductions, the expiration of enhanced premium tax credits and growing pressures associated with Medicare Advantage. More than 40% of rural hospitals operate at a financial loss, and since 2010, more than 150 rural hospitals have closed or converted, reducing access to care for many patients, according to data highlighted in the report.

“Rural hospitals are precious pillars of community life, delivering essential care, rewarding jobs and economic stability for millions of Americans,” said AHA President and CEO Steve Walsh. “The pressures on rural providers are only intensifying and the warning signs have become impossible to ignore. Through urgent and collaborative policy action, we can protect patients in rural communities and ensure they continue to get the care they need close to home, when and where they need it most.”

The report outlines key policy priorities, including:

  • Reversing harmful Medicaid cuts.

  • Directing Rural Health Transformation Program funds to rural hospitals.

  • Modernizing and strengthening rural payment models.

  • Preserving and expanding telehealth access.

  • Addressing harmful corporate insurer practices.

  • Investing in the rural workforce.

  • Protecting access to the 340B Drug Pricing Program.
