AHRQ challenge seeks digital solutions to aid hospital-to-home transitions

Researchers and others may apply through April 6 to participate in a $175,000 competition to develop innovative digital solutions to help manage the transition from inpatient hospital care to home, especially for patients with multiple chronic conditions, low health literacy or limited English language proficiency, the Agency for Healthcare Research and Quality announced last week. The challenge seeks personalized, high-tech ways to ease administrative and information management burdens and support patient engagement. Semifinalists will develop a prototype of their proposed solution.  

Mathematica recently won an AHRQ challenge to develop tools that help visualize and share community-level data on the social determinants of health.
