Rural health networks can apply through April 6 for a portion of $1.6 million in funding to support care coordination strategies to improve outcomes for patients with heart failure, chronic obstructive pulmonary disease and diabetes, the Health Resources and Services Administration announced last week. The Rural Health Care Coordination Network Partnership Program will award up to $200,000 per year for three years to formal rural health networks consisting of least three health provider organizations.
