The AHA and its Health Research & Educational Trust affiliate are surveying hospitals through July to catalog their efforts to improve hospital discharge transitions as part of Project ACHIEVE, a study funded by the Patient-Centered Outcomes Research Institute to evaluate the effectiveness of transitional care. The survey should take about 15-20 minutes for a hospital staff member in case management, quality improvement or administration to complete with help from clinicians.
