Clifton-Fine Hospital in St. Lawrence County, N.Y., adjacent to the Canadian border, was long challenged by low patient numbers and an outdated, cramped emergency room so small and crowded that staff struggled to treat multiple patients with space or privacy. The facility faced the real possibility of closure. To remain viable, the hospital became New York state’s first designated Rural Emergency Hospital — a federal program that trades inpatient services for guaranteed annual funding to support outpatient and emergency care.

In 2026, Clifton-Fine opened its new $20‑million emergency department, built entirely with grant dollars separate from its federal designation. The modern facility offers vastly expanded space, state‑of‑the‑art technology on par with larger regional hospitals, private rooms, observation and triage areas, radiology services, and a new CT scanner.

Hospital leaders say these upgrades will help the institution stay sustainable and relevant for decades to come — critical for a community whose nearest alternative hospital is an hour away. The original building will be renovated for lab and physical therapy services, and Clifton-Fine hopes its transformation will serve as a model for other rural hospitals navigating similar challenges.
