Description

Developing an integrated population health strategy that integrates care model design with community interventions is essential as hospitals and health systems drive value and improve health equity.  Population health tools and methods can be used to better understand the needs and strengths of patients and communities, and develop system-level strategies to impact positive change. In this virtual workshop, presenters from AHA and the Institute for Healthcare Improvement will review the Pathways to Population Health framework and how it can help you develop a more targeted, integrated and efficient population health strategic plan.
