The Centers for Medicare & Medicaid Services today announced a four-year extension of its Initiative to Reduce Avoidable Hospitalizations among Nursing Facility Residents, which will allow current participants to apply to test a new payment model. Since 2013, the initiative has partnered with seven organizations to implement evidence-based interventions aimed at reducing hospital readmissions for long-stay residents of long-term care facilities who are dually enrolled in Medicare and Medicaid.
