The Physician-Focused Payment Model Technical Advisory Committee recently submitted four recommendations to the Department of Health and Human Services, including a recommendation that the agency test the AHA-supported Advanced Care Model to support people with advanced illness. The Coalition to Transform Advanced Care developed the ACM with input from AHA and its members, who also voiced support for the model to the committee. The panel also recommended that HHS test a second advanced illness model and implement models for providing acute care in the home and telehealth care management in skilled nursing facilities. The Medicare Access and CHIP Reauthorization Act of 2015 created the advisory committee to recommend physician-focused payment models to the HHS secretary.
