Cleveland Clinic is bringing its Hospital Care At Home program to Northeast Ohio, offering eligible patients the option to receive hospital-level acute care without leaving their homes. The initiative is available to patients within a 25-mile radius of Fairview and Avon hospitals and offers treatment for conditions such as pneumonia, asthma, sepsis and chronic obstructive pulmonary disease through a blend of virtual monitoring and in-person clinical visits. Patients are equipped with remote monitoring devices, emergency response tools and direct connections to a 24/7 virtual care team through the hospital’s centralized command center. “Giving patients the option to recover in the setting that best meets their individual needs is essential,” said Neil P. Smith, D.O., president of Cleveland Clinic’s West Submarket.

The program builds on Cleveland Clinic’s earlier success in Florida, where Hospital Care At Home helped more than 4,000 patients recover while reducing hospital readmissions. In Ohio, care teams coordinate closely with patients’ primary physicians and deliver services such as lab work, imaging and IV therapies directly in the home, ensuring continuity and quality of care. “Hospital Care At Home is part of our ongoing commitment to evolving the way we deliver care,” said Jorge A. Guzman, M.D., executive vice president and president of Cleveland Clinic’s Northeast Ohio Market. “We recognize that for many patients, healing at home can support their recovery journey.”
