Healing at Home: NewYork-Presbyterian Expands Care Beyond Hospital Walls

NewYork-Presbyterian announced the launch of its Hospital-at-Home program in November, designed to provide hospital-level care to eligible patients in the comfort of their homes. The initiative aims to create a positive healing environment while maintaining the same high-quality standards as in-hospital care.
To qualify, patients must be medically cleared by a physician, live within 30 minutes of either of NewYork-Presbyterian’s two medical campuses in Manhattan, and have a clean, accessible home. Participation is voluntary and covered by Medicare. Services include remote monitoring, imaging, pharmacy support, and both in-person and telehealth visits, ensuring comprehensive care without requiring a hospital stay.
Once enrolled, patients receive a device to monitor vital signs and benefit from twice-daily home visits by care team members for medication and progress checks. Daily telehealth consultations with a medical provider complement the visits. The program also focuses on smooth transitions post-discharge, helping patients and families plan follow-up appointments, adjust medications, and recognize warning signs to maintain health after completing the program.
