Trinity Health’s farm program brings Food as Medicine to community tables

Trinity Health Michigan’s The Farm has been connecting healthy eating with better health outcomes for community members since its start in 2010. Located on hospital campuses in Ypsilanti, Pontiac and Muskegon, The Farm grows fresh vegetables year-round for patients and community members facing food insecurity. The program also offers education and resources to address health disparities and improve access to affordable whole foods.
The program’s work is rooted in the belief that nutrition plays an important role in preventing and managing chronic disease. By investing in regional food systems and expanding access to fresh foods, The Farm helps strengthen nutrition security across Michigan communities through partnerships, community outreach and innovative farm-based initiatives.
