Rethinking Hospital Community Benefit at the Neighborhood Level to Improve Resident Health

Learn how Saint Francis Memorial Hospital is partnering with Saint Francis Foundation to play a deeper role in improving health outcomes for residents in the Tenderloin, a highly challenged neighborhood adjacent to the hospital that is home to many high users of health care. Saint Francis launched the Tenderloin Health Improvement Partnership (TLHIP) in 2014 with the goal of improving community health by addressing social determinants.

An early adopter of the collective impact framework, TLHIP has forged a partnership with 100 government agencies, nonprofits, business and grass roots organizations. This partnership is working together with residents to co-create pioneering population health solutions focused on improving neighborhood safety, building community connection, and creating opportunities to make healthier choices. These approaches have led to changes in how community stakeholders work together, enabling more effective collaboration and alignment of efforts for greater impact. TLHIP believes that improving neighborhood conditions will lead to an improved system of health and wellbeing for residents, ultimately reducing preventable emergency department visits and hospital readmissions.
