Patients’ non-medical needs impact their health and contribute to health inequities. Hospitals and health systems are identifying best practices for social needs screening, and collecting and connecting patients with resources; and health care professionals have an important role to play in that process. Meeting patients’ social needs is a collaborative effort, which requires engagement of various professions from across the health care spectrum and community.
