Medicare’s payment differences between hospital outpatient departments (HOPDs) and physician offices reflect the reality that HOPDs care for more complex patients, operate under stringent safety requirements, and maintain vital community services. But with hospitals already absorbing significant underpayments — receiving just 82 cents for every dollar of Medicare care — further site-neutral cuts would accelerate service reductions and threaten access in the very communities that rely on hospitals most.
