The Centers for Disease Control and Prevention’s Division for Heart Disease and Stroke Prevention is recruiting health systems and health care providers to pilot test health equity indicators for cardiovascular health. Participating sites will contribute to a publicly available toolkit that provides guidance and examples of applying health equity approaches in the prevention, treatment and intervention of cardiovascular disease. To suggest a potential site or request additional information, email healthequitypilot@cdc.gov by Nov. 5.
