Hartford Foundation have announced a new award to recognize hospitals and health systems that are transforming care for adults 65 or older: the AHA Age-Friendly Healthcare Impact Award. Applications open Aug. 3 and are due Sept. 25. We invite you to support the AHA and JAHF in promoting this new award.
All U.S. hospitals and health systems that have at least two settings of care where the 4Ms — What Matters, Medication, Mentation and Mobility — are being delivered and have earned level 2 recognition as an “Age-Friendly Health System — Committed to Care Excellence” are eligible to apply for this award.
