Improving Nutrition for Seniors
Adirondack Health is working with multiple sectors of the community, such as the Franklin County Office for the Aging and the New York State Agriculture & Market Department to provide eligible, low-income seniors with coupons to local farmers markets. Seniors can use these coupons to purchase fresh produce and fresh-cut herbs. This initiative not only promotes healthy eating among seniors by connecting them to nutrition-assistance programs and healthier food options across the community, but also supports the establishment of farmers markets and local businesses, such as roadside stands and community-sponsored agriculture programs. Services like these increase access to healthier foods, impacting the health and well-being of community members.
This case study is part of the HANYS Triple Aim series highlighting how New York hospitals are improving health, enhancing quality and reducing costs. Hospitals, nursing homes, and home-care agencies across New York state are pursuing the Triple Aim. In spite of fiscal constraints, its members are embracing the challenge of transforming health care and are implementing new and innovative approaches to delivery.
