Join us for an inspiring and practical webinar featuring Brittney Daley as she shares Morristown Medical Center’s age-friendly journey. This session will highlight real-world innovations, measurable outcomes, and actionable next steps that have helped advance 4Ms care (What Matters, Medication, Mentation, and Mobility).

Participants will gain insight into how to identify where the 4Ms already exist within current systems and workflows, develop a clear 4Ms care model, and establish meaningful data tracking strategies. Whether you are refining existing efforts or building momentum across your teams and organization, this session offers practical approaches and valuable perspectives to advance age-friendly care.

Attendees will leave with actionable ideas, proven strategies, and inspiration to strengthen and sustain their 4Ms implementation.

Learning Objectives:

  • Identify where the 4Ms framework is already embedded within existing clinical workflows and systems.
  • Describe key steps in developing a clear and actionable 4Ms care model within their organization.
  • Understand how to select and track meaningful data to measure age-friendly care implementation and outcomes.
  • Apply lessons learned from Morristown Medical Center to inform early-stage implementation strategies.
