Does your organization have an innovative palliative and end-of-life care program? If so, consider applying for the 2018 Circle of Life Award.

Since 1999, the “Circle of Life Award: Celebrating Innovation in End-of-Life Care” has honored shining examples of human compassion – care that helps people feel dignified, worthy and loved.

The award pays tribute to the hospitals, physicians, nurses and others who are leading the way toward better practices in providing care at the end of life. They are making inroads into one of the most difficult aspects of health care by stretching the current boundaries of who is served, how services are integrated into a seamless continuum, and how palliative and end-of-life services mesh with traditional medical treatment.

The Circle of Life Award honorees approach palliative and end-of-life care in unique ways that reflect both their organizational culture and community needs. All have experiences and ideas to share with the rest of the nation’s hospitals, hospices, and long-term care facilities. Their accomplishments enrich and inspire all of us, and their commitment touches lives everywhere.

The recipients of the 2017 Circle of Life Award will be recognized July 28 at the 25th Annual Health Forum/AHA Leadership Summit in San Francisco. Aug. 14 is the deadline to apply for the 2018 award. Visit www.aha.org/circleoflife or email circleoflife@aha.org. You can hear past honorees talk about their innovations and the award’s value.

And plan to attend the Leadership Summit. Join your colleagues from across the country to discuss what it takes to succeed in this changing health care environment.

Registering for the event is as easy as clicking on the Leadership Summit icon at www.aha.org
