This webinar will introduce the AFHS movement and the 4Ms Framework, What Matters, Medication, Mentation, and Mobility, and explore how aligning AFHS with broader efforts can strengthen care delivery. Learn how Novant Health used data, team-based strategies, and quality improvement principles to drive adoption, overcome challenges, and build momentum for culture change in the care of older adults.

- Describe the core components of the Age-Friendly Health Systems initiative and the 4Ms Framework
- Explain how team-based strategies can support implementation of age-friendly care across hospital settings
