Mental Health Assessment and Triage Center:  Reducing ED Volume and Improving Mental Health Access

Agenda:

The KC Mental Health Assessment and Triage Center is a public and private partnership between Kansas City stakeholders including hospitals, courts, law enforcement, city officials and the Department of Mental Health.  This successful enterprise diverts persons with mental health and substance use disorders away from area emergency rooms and jails to a safe place where they can be assessed and stabilized.

Super-utilizers: Using Data and Innovation to Treat Our Most Vulnerable

Agenda:

"Super-utilizers" refers to a small section of the population accounting for a large portion of emergency room visits and puts a heavy burden on hospital services and public health budgets. These types of patients can be especially challenging as they are often dealing with homelessness, addiction and mental illness. Our speakers-- Chris Echterling, M.D., medical director, Bridges to Health and Healthy York (PA) Network, WellSpan Health and associate medical director for Quality and Innovation of the WellSpan Medical Group, John Anderson, president and CEO, Anderson Regional Health System, Meridian, MS and Casey Hendricks, J.D., vice president, The Montgomery Institute, Meridian, MS-- led the discussion on how using the data available has allowed hospitals to reduce ED visits, address pressing mental health needs and create greater wellness in a historically unhealthy population.

Webinar Series: AHA Task Force on Social Determinants of Health

Agenda:

In our second of four free webinars, Priya Bathija, AHA Policy reviewed the strategies for the social determinants of health. Guest speakers included Ken Shaffer, M.D. and Abby Olson, director, CHI Health Good Samaritan, Kearney, Neb. and Andrew Orefice, program coordinator, and Augusta Mueller, community benefits manager, Yale New Haven Hospital, Conn. who shared their strategy with the community on issues such as housing, transportation, food and other community health needs necessary to advance population health. Participants shared their experiences and discussed steps to address social determinants in their communities.

Webinar Series Part I: AHA Task Force Telehealth/Virtual Care

Agenda:

In our first of four webinars, Priya Bathija, AHA Policy reviewed the strategies for virtual care/telehealth. Joining her was Janice Favorite, Senior Director, Strategy & Business Development who shared the experiences of the Sacramento-based Dignity Health Telemedicine Network and Bryan Slaba, CEO, Wagner Community Memorial Hospital, South Dakota who is a member of AVERA Health. They discussed the use of telemedicine in emergent settings such as hospital EDs as well as the use of telemedicine in clinics to advance population health. They also reviewed behavioral telehealth and relationships with Rural Health Clinics.

Emergency Department Community Partnership

AGENDA

Monmouth Medical Center (MMC) is a 527-bed community hospital affiliate of RWJ Barnabas Health and Monmouth Family Health Center (MFHC) is a Federally Qualified Health Center pursuing recognition as a Patient Centered Medical Home. Together they improved access and reduced costs through an ED case management program that enhanced care coordination while ensuring a medical home for an otherwise vulnerable population.
