Eliminating Harm Checklists

A key component to making patient care safer is to track your data and progress towards improvement. The HEN 2.0 project, supported by Centers for Medicare & Medicaid Services (CMS) Partnership for Patients (PFP), provides a number of tools to support hospital leaders. This compilation of checklists is part of a larger change package on the topic, which include a compilation of evidence-based best practices, improvement strategies and action items along with checklists and resources that offer approaches that may be effective within your organization.
