QCHI’s Health Facts Cards have been distributed to community members in English and Spanish. Individuals are encouraged to record information about their medications, vitamins, herbal and nutritional supplements, allergies, health history and emergency contact information.

QCHI’s Health Facts Cards have been distributed to community members in English and Spanish. Individuals are encouraged to record information about their medications, vitamins, herbal and nutritional supplements, allergies, health history and emergency contact information.
Who is it for?
Residents of the Quad Cities.
Why do they do it?
Every individual can be the center of his or her health care team by understanding their medications and medical treatment, and sharing that information with all of their care providers.
Impact
Since 2008, the program has distributed some 250,000 Health Facts Cards to community members. The free cards are available at all Genesis and Trinity sites, as well as at physician offices and pharmacies throughout the community.
