In this webinar, nurse leaders from Texas Health and Lee Health share their organizations' care innovation and transformation stories. Texas Health, in North Texas, and Lee Health, in Southwest Florida, each have sponsored multiple teams from their organizations over seven CIT cohorts and 10 years of the CIT program. The webinar provides unique insights into the success of the CIT program within these health systems and how it helped drive a value-based care strategy.

Texas Health and Lee Health have been partners with the CIT program since its earliest inception in 2007-2009. Texas Health Resources is a nonprofit organization that operates a network of 24 hospitals and related health facilities in the North Texas region of the United States. Lee Health is one of the largest health care systems in Southwest Florida with over 100 locations, including four hospitals, and offers doctors and medical services throughout Lee County.
