Froedtert Health Community Memorial Hospital - Community Outreach Health Clinic

The Community Outreach Health Clinic provides free care to uninsured and underinsured patients in the Menomonee Falls area. The clinic is open two evenings and one afternoon each week, and is staffed by volunteer physicians, nurse practitioners, nurses, and other clinical and clerical volunteers. The clinic serves patients who don’t have anywhere else to turn for health care services, with the majority of the patients seeking care related to chronic illnesses such as diabetes and hypertension. The Community Outreach Health Clinic is funded by in-kind and volunteer support from Froedtert Health Community Memorial Hospital and donations from community foundations, businesses, and individuals. Patients may choose to make a modest donation in exchange for care received.

Who is it for?

Uninsured and underinsured patients that live in the surrounding area with a total income at or below 185 percent of the federal poverty level.

Why do they do it?

In the late 1990s, Froedtert Health Community Memorial Hospital conducted a community needs assessment, and identified the need to provide access to health care for the working poor in the Menomonee Falls area. The Community Outreach Health Clinic was established in 2000 in response to the need for quality, affordable health care for those without other resources.

Impact

The Outreach Clinic is clearly meeting a community need, as evidenced by the steady growth in patient visits. Every year, the number of patients treated at the clinic has increased; in 2012, the clinic had more than 2,550 patient visits.
