Readiness for an Award
How do you know if your hospital or health system is ready to apply?
- The program described in the application must be operational for at least 2 years.
- The program must have demonstrated positive impact in the community, and documentation of or data on the program’s effectiveness is available.
- The program is sustainable and can be replicated in other communities.
- The program improves equity in your community.
