Convenient Care For U.S. Veterans

North Shore-Long Island Jewish Health System opened a facility to treet heavioral health and primary care services for veterans and their families all under one roof. Working with the U.S. Department of Veterans Affairs, the center brings together specialists from both Long Shore-Long Island Jewish Health System and the VA to collaborate to meet the needs of these service members and their familes.

Working with the U.S. Department of Veterans Affairs, the center brings together specialists from both Long Shore-Long Island Jewish Health System and the VA to collaborate to meet the needs of these service members and their familes.

Services offered include:

  • Treatment for children, adolescents, adults, couples and families
  • Customized treatment plans, medication therapy and psychotherapy (individual, group and family)
  • Health and wellness management
  • Education and supportive resources
  • Parenting and child workshops
  • Outreach, presentations, consultation and crisis counseling for military agencies

This case study is part of the NYS Triple Aim series highlighting how New York hospitals are improving health, enhancing quality and reducing costs. Hospitals, nursing homes, and home care agencies across New York State are pursuing the Triple Aim. In spite of fiscal constraints, its members are embracing the challenge of transforming health care and are implementing new and innovative approaches to delivery.
