Post-acute care providers play an essential role in ensuring that patients receive the care they need to heal and have a smooth transition back to a community-based setting, typically after a discharge from a hospital. These providers face an increasingly complex regulatory and market environment as health care transforms from a system that rewards volume to one that encourages and rewards value.
