Recent Achievements:

• Bluegrass Care Navigators continues expanding transitional care services to meet the needs of our communities. In the past year, Bluegrass Care Navigators began partnering with the city of Lexington and local hospitals to support homeless individuals who are high utilizers of emergency departments. The goal of the program is to provide stable housing, recovery, and increased quality of life for the homeless in the community.
• Bluegrass Care Navigators is in the inaugural cohort of leading health care organizations joining Ariadne Labs, the Center to Advance Palliative Care, and VitalTalk in a pioneering new collaborative to transform care for individuals facing serious illness. Working with the University of Kentucky, this collaborative focuses on implementation strategies, tools, and support to improve communication between clinicians and seriously ill patients.
• Bluegrass Care Navigators enhanced services in our physician practice to include home-based primary care.

Organization: Providence TrinityCare Hospice & TrinityKids Care, Providence Little Company of Mary Medical Center Torrance, and Providence Institute for Human Caring, Torrance, California. Innovation highlights were its seamless alignment, whole-person care, and data-driven approach.
