The Value of an Organized Fracture Program for the Elderly: Improved Outcomes and Reduced Costs

Highland Hospital's Geriatric Fracture Center has achieved improved clinical outcomes (complications, length of stay, mortality, and readmission rates) and lower costs using a model of co-managed care. The patient population includes those age 60 and older with fragility fractures, who are medically/surgically complex. The model is based on patient centered, evidence-based care delivered by a collaborative interdisciplinary care team. A standardized approach is taken from admission to discharge, using standard order sets, a goal of early surgery, and consistent post-operative management.
