Case Study: Making Age-Friendly Care a Mission Goal

On its journey to become an Age-Friendly Health System, St. John’s Riverside Hospital formed a multidisciplinary team to implement the 4Ms: what matters, medications, mentation and mobility. In addition, the hospital’s IT team developed a centralized dashboard for every patient age 65 and older called the Age-Friendly Clinical Panel, which serves as a 4Ms data hub for all teams.
