HANYS' 2015 Awards
The Heathcare Association of New York State announced its 2015 Community Health Improvement award winners and the 2015 Pinnacle Award for Quality and Patient Safety.
2015 Community Health Improvement Award
Bassett healthcare Network received the award for its school-based health/oral health program. The program offers preventive care at 19 school-based health centers and extensive restorative services at three of the centers. In 2014, the program provided preventiver services for 2,160 students and restorative care to more than 400 patients. View further information about the program.
Columbia Memorial Health and Montefiore New Rochelle received honorable mentions for their programs. Columbia provides a mobile dental health services in 2 counties at 14 different schools. Montefiore's program is preventing and reducing childhood obesity with a 90 percent participant completion rate with 61 percent of children lowering their body mass indexes. Read their stories today.
2015 Pinnacle Award for Quality and Patient Safety
The Silvercrest Center for Nursing and Rehabilitation program to reduce hospitalization of long-term, chronic critically ill patients was awarded for their achievement in reducing hospital transfers, admissions and readmissions. Additionally, the University of Vermont Health Network--Elizabethtown Community Hospital was honored for its care transition program. Finally, Strong Memorial Hospital was recognized for its innovative multidisciplinary pediatric asthma care team. Read the details the results
