Texas Health Harris Methodist Hospital Azle - Healthy Education and Lifestyles Program (HELP)

HELP provides access to routine health care, culturally competent care and education, and a support group. HELP care plans are customized based on the individual’s needs, and high-risk program participants receive a kit containing an electronic tablet and other necessary medical equipment to monitor their chronic disease. These tablets are wirelessly linked to the HELP care team, who can then monitor participants’ conditions outside the clinic.
