Custom Support for Your HCC Journey

Looking to take your community partnership work to the next level? We offer custom support at different levels to help facilitate your success by using the HCC tools to their fullest potential. This offering lets you engage with coaches and subject matter experts to delve into each of the six HCC modules.

Host Your Own Cohort

Identify a topic and build your own cohort (up to 15 participants) to go through the six HCC modules together. For this option, AHA will provide subject matter expert HCC coaches to lead a private HCC program for your team. This option is best for a system, region or state that is looking to advance their partnerships on a topic of shared importance.

Join an HCC Cohort

In this option, you can apply to join a cohort with peer organizations and go through the six HCC modules together. As in the previous option, AHA will provide subject matter expert HCC coaches to lead an HCC program with other members drawn from applicants responding to an announced opportunity.
